Why Does a 5-Hour Evaluation Change Everything?

Living with Ehlers-Danlos Syndrome (EDS) and its symptoms without a proper diagnosis can be exhausting, confusing and risky. A 5-hour, in-depth evaluation provides the following:

The first hour is gathering information about your condition and how it affects you in your day-to-day life activities. 
-I will review all MRI reports.
-I will gather information about any systemic issues such as MCAS, POTS, GI issues, etc.
-I will gather information regarding any neurological issues, such as: tethered cord, CCI, AAI, Chiari malformation, bulging disc, etc.  
-I will perform neurological screenings for tethered cord, CCI, AAI, and Chiari. 
-I will gather information regarding any orthopedic issues, such as: labral tears, meniscal tears, rotator cuff tears, etc. Subluxations and dislocations will be reviewed in a separate section.
-I will review all information regarding surgeries.
-I am a physical therapist.  Therefore, I cannot comment on any medications you are taking or what to switch to. That is your PCP‘s job.
-I will sit down and explain how all the information I gathered will affect your outcome in physical therapy.  

The next 60 to 90 minutes, I will evaluate and treat your low back, hips, knees, ankles, and feet.  
-I will first perform a postural assessment. 
-I will test for bulging disc, tethered cord, hip labral tear, meniscal tears, etc. 
-I will treat your low back to the best of my ability and try to get your pain level as low as possible, with my goal being 0 out of 10 pain. 
– I will then evaluate your hips, knees, ankles, and feet. With the same goal of 0 out of 10 pain.  
– I will evaluate if a custom orthotic would be appropriate for you and, if purchased from an EDS knowledgeable provider, would decrease your pain.

– I will assess your gait. 
-After we have completed this area, I will explain to you where your pain is coming from and how all these joints interrelate. 

The next 30 minutes are spent recording videos on your phone.
-I will record videos explaining all of the biomechanical issues you were having with your low back, hips, knees, ankles, and feet.  This way, you can review what I told you at home at your leisure.  You can also show the video to your local physical therapist and PCP.  All videos are specifically about you and your issues. 
-I will video any manual techniques that I used during my treatment that decreased your pain so your local PT can use them on you to help you feel better every visit with them.
-I will then instruct your family member on how they can help decrease your pain at home for your low back, hips, and knees.  

The next 60 minutes will be evaluation and treatment of your shoulders, mid back, ribs, neck, TMJ, elbow, wrist, hands, and head PRN, with the same goal of 0 out of 10 pain in all these areas. 
-I will test for shoulder labral tears and rotator cuff tears, as well as CCI, AAI, etc.
-At the end of the evaluation and treatment of these areas, I will sit down and discuss with you how they all interact with each other.


For the next 30 minutes, I will record videos for your local physical therapist on any manual techniques that helped decrease your pain in these areas so they can reproduce the same results that we achieved during our treatment session. 
-I will also record a detailed description of how all of the above body parts interact with each other as it relates to your specific issues. 

In the final 30 minutes, I will sit down with you and discuss whether or not exercises will need to be modified for your specific conditions. We will also come up with a plan that you can follow so you and your physical therapist can go step-by-step to alleviate your pain and allow you to become more functional with all your activities of daily living. 
-Any joints that did not get to 0 out of 10 pain, we can discuss our options such as bracing or surgery. 
-Within two weeks, a detailed note outlining all of your issues will be sent to both you and your PCP, discussing all orthopedic, neurologica,l and systemic issues that need to be addressed by other providers in your local area.

  1. Book ½ hour Facetime appointment 1 month after evaluation to clear up any issues you may have as you begin this protocol.
  2. Book a follow-up appointment with Kevin through FaceTime anytime. Here, Kevin can monitor your progress and problem-solve any roadblocks that may occur during your journey. 
  3. Book regular ½ hour appointments every 2-3 weeks for Kevin to progress your exercises on FaceTime. 
  4. Book 1-hour FaceTime with Kevin, yourself, and your PT at home to review the new manual techniques that Kevin has explained in your videos. 
  5. Come in for a 1-hour live session with Kevin to be aligned again.
